Volunteer Interest Form

Thank you for considering volunteering to help with the Night Shelter. Please fill in the details requested below, and click the button at the bottom. Filling in this form does NOT commit you to work as a volunteer. We shall get in touch with you.

Firstly, please key in a 'username' and 'password'. The username should be a shortened form of your name, and the password should be something you can remember easily. If you become a volunteer with the shelter, you will be able to use the username and password to log on to this website to check your commitments.

Username Password ...and again (so we can check that we got it right)

Do write down your username and password so that you don't forget them!

Now we need some personal details. We will hold them on our database and not pass these on to any other organization. Should you wish your details to be removed, contact us, and we shall delete them once the Night Shelter season has ended (in April). Please note that the reason for asking your gender is so that we can ensure that there is at least one woman and one man on each shift. This ensures that there will always be at least one volunteer who is the same gender as any specific guest. Similarly, if we know the year in which you were born we can try and avoid volunteer teams which consist entirely of young (or retired) volunteers.

Your Church
Wycombe's Winter Night Shelter was originally set up by a group of local churches working together. You do not need to belong to a church to take part, however if you do belong to a church, it is helpful for us to know which one. In this way we can keep your church informed regarding the Shelter. Some of your friends there might want to get involved too! Please select the name of your church from the list below. If you don't belong to a church, or you do not want to answer this question, please choose the appropriate option. If your church is not listed, choose the "not listed" option, and key in the name and town of your church in the box on the right.

Referees
Given the nature of the work, unless you have served with the Wycombe Winter Night Shelter as a volunteer before, we need the names and emails (or addresses) of two people who could give us a reference. If you were a volunteer last year, simply give the name of your Venue Co-ordinator or Shift Leader and the Venue at which you served as your first and only referee (e.g. "Alan Clouston at the HUB").

CRB Checks
If you end up volunteering as an assistant at the Shelter, you will need a valid, enhanced CRB check for working with vulnerable adults. If you don't have one, we can obtain one for you.

Do you have a valid, enhanced CRB check for working with vulnerable adults?

If so, who did it?
and when was it done (please give the date in the format DD/MM/YYYY, so that the 25th of December 2005 becomes 25/12/2005)

First Aid
First Aid training is not required for all volunteers However, if you are trained, it is useful for us to know about it. If you want to be trained in First Aid, we can arrange it.

Do you have a valid, First Aid qualification?

If so, give the title of the certificate you hold?
and when does it expire (please give the date in the format DD/MM/YYYY, so that the 25th of December 2009 becomes 25/12/2009)

Previous relevant experience
Many of our volunteers are completely new to this kind of work. If you have previous experience, please describe it below. You might include aspects of your professional training or work, or details of previous voluntary work if they are relevant. If you have relevant qualifications (other than CRB, First Aid or Food Hygeine certificates) please mention them too.
